Meet with a CHAP Program Surveyor to review the process, answer questions, and align certification goals.

Submit policies and other required documents to validate compliance with the standards.

CHAP experts assess the submission and provide feedback. If additional information is needed, organizations have the opportunity to revise and refine materials, otherwise, we will proceed to the interview phase.

The Program Surveyor will…
Meet with key leadership personnel: This includes reviewing relevant personnel files and clinical records.
Provide an opportunity for leadership to: Present an overview of their program.
Address any questions, concerns, and/or follow‑up items raised by the Program Surveyor.
If additional items need attention, the Program Surveyor will schedule a second and final interview.

Certification is awarded upon successful completion of the documentation and interview phases.
