Age-Friendly Care at Home Learning Modules

CHAP has partnered with The John A Hartford foundation and the Institute for Healthcare Improvement to bring Age-Friendly Care into the community-based healthcare setting. Each of these three modules are approximately 20 minutes in length and have the following learning objectives:

Module 1 objective:  At the completion of this module, the participant will be able to describe Age-Friendly Care at Home

Module 2 objective:  At the completion of this module, the participant will be able to describe the Age-Friendly Care at Home Framework and how each of the 4Ms are applied

Module 3 objective:  At the completion of this module, the participant will be able to explain the value of Age-Friendly Care at Home
