Wisconsin (DHS) Home Health & Hospice License Requirements

Home Health – License Required

CHAP can conduct initial licensure survey (2-step process for accreditation and deemed status for Medicare certification).

For renewal of license – a CHAP comprehensive survey can be utilized as proof of compliance in lieu of a state licensure survey.  The Home Health must provide the department provide the department with a copy of the report by the accreditation organization of each periodic review the organization conducts of the hospice.
