CHAP Appoints Jan Sweat as Senior Vice President, Center for Excellence 

San Diego, CA. – Community Health Accreditation Partner (CHAP) is pleased to announce the promotion of Jan Sweat to Senior Vice President, Center for Excellence

Since joining CHAP in 2022 as Vice President of Learning Solutions, Jan has played an instrumental role in expanding the organization’s educational and professional development offerings. Her leadership has helped drive significant growth across CHAP’s Center for Excellence initiatives, including achieving ANCC Accreditation, expanding on-demand learning opportunities, launching Leadership Certification, introducing Advanced Sales Certification, and growing CHAP’s portfolio of disease program certifications. 

This promotion reflects an exciting new chapter for CHAP’s Center for Excellence and its commitment to supporting providers through education, leadership development, certification, and strategic guidance. Under Jan’s leadership, the Center for Excellence will continue to expand how CHAP partners with organizations across home and community-based care, offering more customized, connected, and practical solutions that help providers strengthen quality, improve performance, and achieve their goals.
