CHAP Announces Call for Volunteers and Nominations for Community Advisory Group on Hospital-at-Home Standards & Accreditation
Arlington, VA. – August 20, 2024 – The Community Health Accreditation Partner (CHAP), a leader in home and community-based healthcare accreditation, is pleased to announce that it is now accepting volunteers and nominations for its Community Advisory Group. This group will play a pivotal role in shaping CHAP’s Hospital At Home Standards and Accreditation program.
As healthcare continues to evolve towards more patient-centric models, the Hospital At Home program stands out as a promising innovation that allows patients to receive acute care services in the comfort of their own homes. Recognizing the growing importance of this model, CHAP is committed to developing comprehensive standards that ensure safety, quality, and excellence in care delivery.

About the Hospital At Home Program
The Hospital at Home model has been gaining traction as a viable alternative to traditional hospital care, particularly for patients with acute conditions such as chronic obstructive pulmonary disease (COPD) exacerbation, heart failure, and acute infections. Supported by Medicare, this innovative approach has been shown to improve patient outcomes by reducing readmissions, enhancing patient mobility, and increasing overall satisfaction with care.
How to Participate
CHAP invites interested individuals and organizations to submit nominations or volunteer themselves to join the Community Advisory Group. Those selected will have the opportunity to contribute to the design and implementation of standards that will guide the future of Hospital at Home services. As a Centers for Medicare & Medicaid Services (CMS)–approved accrediting organization, CHAP surveys organizations providing home health, hospice, and home medical equipment services to establish if Medicare Conditions of Participation and DMEPOS Quality Standard are met and recommend certification to CMS.
